ConcertoCare Website

Context:

ConcertoCare implements an interdisciplinary care model, using tech-enabled, in-home care teams, to address unmet health and social needs and improve patients’ quality of life by partnering with them, their caregivers, families, health providers, and communities. ConcertoCare’s website needs to be updated to better reflect these values and make it easy for potential users to learn about them and use their services.  It needs to include a new contact/enrollment page, and the website needs to merge information across four other subsidiary websites.

Challenge:

  1. Update website to better communicate what ConcertoCare does and how they serve patients.

  2. Make it easier to auto enroll.

  3. Optimize outreach to new patients and caretakers.

  4. Manage new traffic coming in from legacy sites and add new content.

  5. Create cohesive branding.

Solution:

Create a website that merged all the information necessary in an organized, concise way, and make enrollment workable from front-end and back-end.

User Research

I researched other healthcare competitors that focused on in-home care, and was able to gain insight on the best ways to design the website which would make it easily understandable and navigable. I made sure to always keep in mind the user. The target audience is a wide range of users including patients, their families and caregivers, healthcare workers, and anyone interested in learning more about what ConcertoCare has to offer.

In-home care and virtual healthcare are becoming increasingly popular, especially after COVID. Conducting research on the wide range of companies doing this type of care put into perspective the more specific changes that would need to be made to stand out from the crowd.

Pain Points

ConcertoCare had no way of sorting out the different type of users that were trying to get in contact wit them or learn about their services.

There was incomplete information spread across multiple subsidiary websites, and the branding was incohesive due to the aquisition of different companies.

Auto-enrollment is extremely important to gain new patients, and there was no way to auto-enroll on the website.

Design Process

Sitemaps

The entire site needed to be re-organized to include more information, and easier navigation. The process of creating these sitemaps came together through weekly meetings with my team including a developer, and the marketing team. It was up to me to decide first concepts and where to take the direction of the navigation. The sitemap went through four versions before coming to this final version.

'Get In Touch' Page

The Get in Touch page was prioritized, due to ConcertoCare not already having a reliable contact page where they could successfully enroll new patients, and provide information to patients, senior living facilities, or families. Through working with the marketing team at ConcertoCare and the developer we came up with a few different versions and picked one once we made corrections to the wireframes. This is the get in touch page with the different drop down option.

Wireframes

This wireframe was the jumping off point for the organization of the website. There were new features added like FAQs, a new careers page, location page, and reorganized blog page.

Accessibility

Easy to Read

The text can be made larger and has a number of images to accompany the text. People who may have a hard time seeing text will more easily see black text against a white background.

Contrast

The branding colors, and the text colors ran into some issues reaching ADA compliance. This had to be tested throughout the design process to make sure all users were able to read the information on the site.

Final Iteration

Hi-Fi Screen Flow

This is the final iteration of the hi-fidelity prototype. I made changes to the original wireframe design that includes a better information architecture, navigation, as well as the full integration of subsidiary websites, and the ability to easily contact ConcertoCare for more information.

Takeaways

Throughout this project, I learned to:

  • Always consider accessibility.

  • Keep navigation simple and clear titles.

  • Researching competitors can be important to figure out how to stand out.

  • Work collaboratively with a varied team.

After finishing this design there are a few design elements I would change or improve:

  • I would have fleshed out wireframes more so prototype would have less versions.

  • I would conduct usability testing to make sure a variety of users could easily navigate the site.
